Insurance
If you would like to work with me and access insurance benefits, please call the number on the back of your health insurance card to find out what your out-of-network mental health benefits are and confirm that they reimburse for services provided by a Licensed Mental Health Counselor Associate (LMHCA).
Some questions you  may want to ask:
  • Do I have out-of-network coverage for mental health services provided? If yes, is telehealth also included in my benefits?
  • What is my yearly deductible? Has it been met or how much more until my deductible is met?
  • How many sessions per year does my plan cover?
  • How much does my insurance plan reimburse for an out-of-network provider for CPT codes 90837?*
  • Does my insurance plan cover therapy with an LMHCA therapist?
  • What is my co-payment (if applicable)?
  • Do I need prior authorization?
  • Do I need approval from my primary care provider?
  • What is the home and mailing address on my file? (important to ensure your checks are issued to the correct address)
  • How do I submit a super bill? Is there an online portal or do I have to mail or fax a copy?
  • What is the time limit to submit a super bill?
*CPT codes refer to the type of service you received.  CPT code 90837 refers to a clinical 60 minute session (a clinical hour is typically 53 minutes to allow time for documentation).

Payment is collected directly after each session.  I will text you a link to an app called "Ivy Pay". It is a HIPPA compliant and secure app that allows you to put a credit card or an HSAFSA card on file. You enter it one time and then I just charge it after each session. If you prefer to pay by check or cash, it will need to be collected at the time of the session.

A limited number of sliding scale spots are available.
