Intake Session (first session, 55 minutes): $185

Insurance - Turning in a Superbill
1. Get the Superbill
  • Ask your therapist or provider for a superbill (an itemized receipt).
  • It should list your name, provider’s information, diagnosis code(s), treatment code(s), dates of service, and payment amount.
2. Call Your Insurance Company
  • Look on the back of your insurance card for the member services or claims number.
  • Ask:
    • “Do you accept superbills for out-of-network reimbursement?”
    • “Where do I send a claim?”
    • “Do I need a special form?”
  • Confirm the mailing address, fax, or online portal for claim submission.
3. Complete the Claim Form (if required)
  • Many insurance companies require you to fill out a member claim form (downloadable from their website or portal).
  • Fill in your personal information (name, DOB, policy number).
  • Attach the superbill as the proof of service and payment.
4. Submit Everything
  • Send the superbill with the claim form (if needed) via:
    • Insurance online portal upload
    • Fax
    • Mail
  • Keep copies or screenshots for your records.
5. Track the Claim
  • Most insurers take 2–6 weeks to process.
  • You’ll receive either:
    • A check or direct deposit if you’re reimbursed.
    • An Explanation of Benefits (EOB) showing what was covered or denied.
6. Follow Up if Needed
  • If you don’t hear back by the expected time, call the insurance company with your claim reference number to check status.
  • If denied, ask for the reason and whether you can appeal.
Payment is collected directly after each session.  I will text you a link to an app called "Ivy Pay". It is a HIPPA compliant and secure app that allows you to put a credit card or an HSA/FSA card on file. You enter it one time and then I charge it after each session. If you prefer to pay by check or cash, it will need to be collected at the time of the session.
